Patient's Responsibilities
You are responsible for:
- Following all rules and regulations posted within the health center and on the website.
- Arriving as scheduled for appointments, and notifying the health center at least 24 hours in advance, in case of a cancelled appointment. After 2 No Shows, a $5. penalty will be charged before a subsequent visit, and for each no show thereafter.
- Providing Accurate Information about your medical history.
- Asking Questions, if you do not understand the explanation of your diagnosis, treatment, or any instructions.
- Following Instructions concerning medications and treatment plan, and following up, notifying the clinician of any problems in following instructions.
- Providing information necessary to process insurance claim forms, and to pay any excess charges beyond the limits of your coverage.
- Payment of any charges billed to you. An additional $35. charge is added to any checks returned due to irregularities, such as insufficient funds.
- Being considerate and respectful of the rights of others, patients and health care personnel. Talking or taking photographs on a cell phone is not permitted in the health center.
